Q-Switched Laser

Specialized guide to dermatology and beauty

Comprehensive principles of Q-Switched Laser treatment

Review of photoacoustic mechanism, clinical applications in pigmentation treatment, tattoo and pigmented mass removal, comparison of wavelengths and safety protocols for different skin types.

1. The nature of QSwitch technology and the physics of operation

Q-Switch (Quality-Switched) technology is the gold standard in the treatment of pigmented lesions and external skin inks. Unlike traditional lasers that use pure photothermolysis energy (slow heating), Q-Switch is designed based on the photoacoustic phenomenon (mechanical shock waves). Inside the optical resonator of these systems, there is an ultrafast optical shutter that condenses the beam energy until it reaches a critical threshold and then discharges it in a fraction of a nanosecond.

When a high-energy pulse with a bandwidth of 3 to 20 nanoseconds is delivered to the skin, the irradiation time is shorter than the Thermal Relaxation Time (TRT) of the melanosomes or tattoo pigment particles. The result of this time difference is the breakdown of the pigment structure into microscopic particles without thermal damage to the keratinocyte tissue and surrounding dermis. These fragmented particles are phagocytosed by macrophages of the immune system in the controlled inflammatory phase and eliminated through the lymphatic system.

2. Types of wavelengths and generating crystals

Selecting the right wavelength is central to clinical success with Q-switch. Target chromophores (melanin pigments or various ink spectra) have specific absorption spectra:

Q-switch Nd:YAG (wavelength 1064 nm)

Best penetration depth, lowest absorption by surface melanin, ideal for removing dermal pigmentation and dark inks (black, dark blue, navy) with the highest safety in dark skin types (Fitzpatrick 3 to 5).

KTP dual frequency Qswitch (wavelength 532 nm)

It is obtained by passing a wavelength of 1064 through the KTP crystal. It has very high absorption by melanin and surface hemoglobin; especially for freckles, sun spots, and red, orange, and purple pigments.

Ruby QSwitch (Ruby - 694 nm)

Very effective on melanin pigments and green and blue tattoos, but there is a possibility of hypopigmentation due to the strong absorption of melanin in the above skin types.

Alexandrite Q-switch (Alexandrite - 755 nm)

The wavelength of choice for permanent green tattoos and deep dermal blemishes such as nevus of Uta, with an optimal balance between melanin absorption and penetration depth.

Crystal and system Wavelength Main therapeutic goal Risk for dark skin
Q-Switched Nd:YAG 1064 nm Black/red tattoos, melasma, deep spots Very low (safe)
KTP (Nd:YAG Doubled) 532 nm Red tattoo, freckle, superficial lentigo Medium to high (low energy requirement)
Q-Switched Alexandrite 755 nm Green and dark colors, resistant stains Medium
Q-Switched Ruby 694 nm Light green colors, dermal moles High (risk of permanent white spots)

3. Main clinical and cosmetic applications

In addition to tattoo removal, QSwitch is the primary tool in dermatology clinics for a wide range of congenital and acquired pigmentary abnormalities:

  • Ephelides and Solar Lentigines: Epidermal lesions that show the fastest and clearest response to 532 nm pulses or mild 1064 nm fluences and are usually cleared with 1 to 3 sessions.
  • Nevus of Ota and Nevus of Ito: Congenital gray-blue dermal pigmentation that was historically resistant to other methods, but gradually lightens and clears with the 1064 wavelength.
  • Becker's Nevus: A pigmented hamartoma with hair, in which multi-stage treatment with Q-switch is effective in reducing the pigmented portion.
  • Skin rejuvenation and pore tightening: through stimulation of fibroblasts and type 1 collagen formation in the reticular layer using fractional handpieces without causing superficial scarring.

4. Specialized tattoo removal protocols

Ink removal requires an understanding of multiple physical variables. The initial tissue response to Q-switch radiation is the formation of a phenomenon called frosting , a temporary whitening of the skin surface caused by the formation of tiny bubbles of gas and water vapor due to the photoacoustic collapse of carbon particles.

Ultrasonic Cavitation

Kirby-Desai Scale: A standard tool for predicting the number of tattoo removal sessions based on 6 factors: skin phototype, tattoo location on the body, pigment volume and concentration, degree of ink layering, presence of previous tissue scarring, and color spectrum diversity.

Multiple protocols (R20 and R0):

In the standard protocol, each session is repeated every 6 to 8 weeks to allow macrophages to clear the particles. However, in the R20 protocol , four consecutive sessions are performed in one day at 20-minute intervals (after the previous frosting has subsided), which accelerates tattoo removal in fewer sessions. The more modern R0 protocol uses translucent perfluorodecalin (PFD) emulsions to reduce downtime to zero, allowing for a second and third session without delay.

5. Melasma treatment and Hollywood peeling (Carbon Peel)

Laser Toning for Melasma

Melasma is one of the most challenging pigmentary pathologies caused by vascular dysfunction and melanocyte hyperactivity. High-intensity irradiation of melasma will definitely cause more severe relapse of the spot (Rebound Melasma). The QSwitch solution is the Laser Toning technique :

  • Using a wavelength of 1064 nm with a large spot size (6 to 8 mm).
  • Very low fluence (Sub-thermolytic fluence between 1.5 and 2.5 joules per square centimeter).
  • Destruction of mature melanosomes without killing the mother melanocyte and without inducing severe inflammation.

Carbon laser peel (Hollywood peel)

In this method, a layer of nanocarbon particle lotion is first applied to the skin to penetrate into the open pores and sebaceous glands. Then, a 1064 nm beam is irradiated. The carbon particles act as an artificial chromophore, directing the absorbed heat into the pores and causing a photoacoustic explosion to shrink the pores, gently exfoliate, reduce sebum secretion, and instantly brighten the skin.

6. Pre- and post-care and management of complications

Pre-treatment measures

  • Stop using retinoids and chemical peels 5 days in advance.
  • Absolutely avoid tanning, sunbathing, or solariums for at least 4 weeks.
  • Use of standard lidocaine/prilocaine-based local anesthesia 45 minutes before the shot.

Aftercare

  • Local cooling with cold compresses to reduce erythema and edema.
  • Use healing ointments such as zinc oxide, sucralfate, and anti-inflammatory emulsions until the crusts are removed.
  • Regularly apply a broad-spectrum physical sunscreen (SPF above 50) every two hours.

Management and prevention of side effects

The most common complication in skin with Oriental and Middle Eastern phototypes is post-inflammatory hyperpigmentation (PIH) . To prevent PIH, it is essential to maintain fluence within a safe threshold, carefully cool the skin, and initiate topical tyrosinase inhibitor treatments (such as tranexamic acid, azelaic acid, or arbutin) between sessions. On the other hand, repeated high-energy treatment in melasma can lead to irreversible white spots (Guttate Hypopigmentation), which indicates the complete death of pigment-producing cells, and should be avoided by carefully controlling the number of shots.
